Use this form to register as an Outside Fitness Provider. Select the location of the park where you intend to provide instruction, the day of the week, and the time and date of use.
Please note that this registration does not give you exclusive use of any area requested. Season-long permit holders and residents have priority on all areas.
Items marked with an asterisk (*) are required.
Outside Fitness Providers cannot use the parks on consecutive Sundays
Permittee agrees to defend, indemnify, and hold harmless the City of Greenwood Village, Colorado, its officers, employees, insurers, and self-insured pool from and against all liabilities, claims, and demands including any third-party claim asserted against the City of Greenwood Village, Colorado, its officers, employees, insurers, or self -insured pool on account of injury, loss or damage which arises out of or in any way related to the above described activities whether or not caused by my act, omission, negligence, or other fault or by the act of omission, negligence, or other fault of the City of Greenwood Village, CO, its officers, its employees or by any other cause
I understand by signing this form electronically that I agree to abide by the Outside Fitness Provider Guidelines and will hold a current Greenwood Village Business License.
